Make a childcare receipt in three steps
- Enter your name or your center’s name and address, and your SSN or EIN in the Tax ID field.
- Add a line such as “Childcare for Maya Lopez, 1–31 October” with the amount, and set the payment date and method.
- Download the PDF and give it to the parent; for a year-end receipt, add one line per month or a single line for the year.
What the IRS asks parents to report
IRS Publication 503 requires a parent claiming the Child and Dependent Care Credit to identify each care provider by name, address and taxpayer identification number, and suggests Form W-10 as one way to request it. The parent keeps that information with their tax records rather than sending it to the IRS. A receipt that carries all three from the start is the one parents do not have to chase.

What should a childcare receipt include?
The provider’s name, address and taxpayer identification number (an SSN for an individual provider or an EIN for a business), the parent who paid, the child’s name, the dates or period of care, the amount paid, the payment date and the payment method. The template has a Tax ID field for the provider and a line description for the child and dates.
Why do parents need the provider’s tax ID?
To claim the Child and Dependent Care Credit, IRS Publication 503 says the parent must identify the care provider by name, address and taxpayer identification number. A receipt that already shows all three saves the parent from asking again at tax time. A parent may also use IRS Form W-10 to request it.
Will this receipt work for a dependent care FSA claim?
Most dependent care FSA claims ask for the provider’s details, the dates of care and the amount — which this receipt shows — but each plan sets its own requirements. Check what the parent’s plan asks for, and add any extra detail in the notes.
Should I issue one receipt per month or one per year?
Either works for the parent’s records. Many providers give a receipt with each payment and a single year-end receipt that totals the year, which is the figure the parent needs for the tax return.
